Parenteral nutrition solutions in neonatal brazilian teaching hospital: from indication to administration

Abstract

Aim: The study aimed to monitor and evaluate the steps of nutritional therapy in newborns in a public maternity hospital in Fortaleza, Ceará, Brazil. Method: An observational, prospective and descriptive study was conducted and the data was recorded in structured form according to Decree 272/98 of the Sanitary Surveillance Department of the Health Ministry. Statistical analysis was performed using descriptive statistics of the variables of the quantitative type, using the chi-square test of independence, considering the 5% significance value. Results: Among the 73 newborns followed up, 97.3% had a main diagnosis of prematurity for indication of total parenteral nutrition (TPN). About the requirements of TPN, 2.3% of these were not delivered to the pharmacy service, 16.7% were sent out of the delivery schedule, and 9% had errors in entering data into the computer program to obtain the TPN’s formulas. During the manipulation occurred 0.6% errors in addition and 6.15% in aspiration of substances. At the time of administration, 99% of infants did not have exclusive access to TPN; 98.7% of the bags and 99% of the labels were not checked. In 36%, the site of catheter insertion was not cleaned before administration. Conclusion: The results showed a control of the process involved in nutritional therapy in neonates, but some problems were identified in prescription, manipulation and administration stages that represent critical steps for a successful therapy.
